How long does provider credentialing take?

Published answers to this range from six weeks to six months, which is unhelpful when you are trying to pick a start date. Here is the realistic picture, and what actually moves it.

The short version

Commercial payer credentialing commonly takes 90 to 120 days from a complete application. Medicare enrolment through PECOS typically runs 60 to 90 days. Medicaid varies widely by state, roughly 30 to 120 days. The clock starts when the application is complete, not when it is submitted.

The timelines

These are the ranges in general use. Your experience will sit somewhere inside them depending on payer, state and how complete your paperwork is on day one.

Plan on the upper end. A practice that budgets 120 days and is credentialed in 80 has a pleasant surprise. One that budgets 60 has a cash flow problem.

The phrase that matters: "a complete application"

Every one of those timelines starts when the payer has a complete application, not when you first submitted something. This single distinction explains most of the difference between the published ranges and what practices actually experience.

An application missing one date, one signature or one piece of supporting documentation does not get rejected with a clear explanation. It sits. Often nobody tells you, and the clock you think is running is not.

This is why credentialing frequently takes six months in practice while the published figure is three. The extra time is almost never payer processing. It is the gap before the application was actually complete.
