Accounts receivable is the clearest number in your practice. Days in A/R tells you how long your money sits with somebody else, and the 90+ bucket tells you how much of it you are about to lose. Most practices watch the first number and never work the second.
Claims do not age evenly. Simple claims get paid. What survives past ninety days is disproportionately the complicated, high-value work: denied and never reworked, underpaid against contract, or stalled behind a missing authorization.
That is the bucket with the largest balances and the least attention, because working it is slow and it is always the first thing dropped when the front desk gets busy.
Not alphabetically and not by payer. Highest recoverable value closest to its filing deadline goes first, because that is the money most likely to disappear.
A claim sitting in 90+ is there for a specific reason: denied, pended, underpaid, never received, or waiting on the patient. Each one gets categorized and routed accordingly. Following up without knowing why a claim is unpaid is just calling payers.
This is the one most practices never check. We load your contracted rates and compare payments and adjustments at CPT level, so an underpayment is caught as an underpayment rather than banked as payment in full. A claim paid at 80% of contract looks identical to a paid claim on your aging report.
Once insurance has adjudicated, the remaining balance needs a clear statement and a real contact path. We run a dedicated line for patient billing questions so your front desk is not fielding them.
If your current biller cannot produce these, that is itself the answer to whether your A/R is being worked.
Send your current aging report. We will read it and tell you what we would work first, what looks recoverable, and what has already passed its appeal window. No commitment, and if your A/R is clean we will tell you that.
